Free OT Self-Screening Tool

As parents, we all want the reassurance that our children are thriving in every area of development — from their ability to focus in school, to their coordination on the playground, mental health, and ability to manage their emotions.

That’s where our Free OT Self-Screening Tool comes in!

What is an OT Self-Screening?

A self-screening is a simple, parent-completed questionnaire designed to help you notice patterns in your child’s skills, behaviors, and daily routines. It’s not a diagnosis, but rather a guide to help you:

  • Recognize strengths and areas that may benefit from support.

  • Gain insight into your child’s motor skills, sensory processing, attention, and self-regulation.

  • Make more informed decisions about whether an occupational therapy evaluation could be helpful.

Why This Matters

Early identification and support can make a big difference in a child’s confidence, learning, and overall well-being. Whether your child is entering school for the first time, adjusting to a new grade, or navigating everyday routines, understanding their needs is the first step in helping them thrive.

How It Works

  1. Download the free self-screening form

  2. Identify potential areas needing support by completing the checklist in the areas of sensory processing, social skills, emotional regulation, attention, and coordination.

  3. If you recognize 2+ points of concern, it is possible that your child might benefit from occupational therapy!

If your results indicate potential challenges and would like to discuss these with our occupational therapist, schedule a free consultation call where we can discuss any concerns you are having and offer information on next steps — including free resources, strategies you can try at home, or a more detailed occupational therapy assessment if necessary.
